InDOT has posted original plans for this concrete arch here: http://www.in.gov/dot/div/contracts/letting/archive/2010/may... listed under R-30986. According to the original plans, the bridge had concrete paneled railings and replaced what appears to be a Burr arch covered bridge. Subsequent rehab plans from the 1960s and 1980s suggest that this bridge has been extensively altered.
